Over the last few weeks, the coronavirus disease 2019 (COVID-19) pandemic has marched relentlessly westward. WHO stated that Europe was now the centre of the pandemic? A few days later, deaths in Spain surpassed those in Italy and China. Iran and United Kingdom had also reported over 30,000 deaths as of last week of April, and many other European countries and the USA reported increasing numbers of cases, heralding an imminent wave of fatalities. Following the sweep of COVID-19 is a series of dramatic containment measures that reflect the scale of the threat posed by the pandemic. Lockdowns that seemed draconian when instigated in Wuhan only 2 months ago are now becoming commonplace. However, many countries are still not following WHO's clear recommendations on containment (widespread testing, quarantine of cases, contact tracing, and social distancing) and have instead implemented haphazard measures, with some attempting only to suppress deaths by shielding the elderly and those with certain health conditions. The initial slow response in countries such as the UK, the USA, and Sweden now looks increasingly poorly judged. As leaders scramble to acquire diagnostic tests, personal protective equipment, and ventilators for overwhelmed hospitals, there is a growing sense of anger. The patchwork of harmful initial reactions from many leaders, from denial and misplaced optimism, to passive acceptance of large-scale deaths, was justified by words such as unprecedented. But this belies the damage wrought by SARS, Middle East respiratory syndrome, Ebola virus, Zika virus, the 2009 H1N1 influenza pandemic, and a widespread acceptance among scientists that a pandemic would one day occur. Hong Kong and South Korea were tested by these previous emerging infections, leaving them better able to scale up testing and contact tracing. Globally, many people are afraid, angry, uncertain, and without confidence in their national leadership. But alongside these dark sentiments, images of solidarity have emerged. Health workers have shown an incredible commitment to their communities and responded with compassion and resolve to tackle the virus despite challenging and sometimes dangerous conditions.

Key Highlights

The number of cases in the US has above 1,000,000 while New York is fast approaching towards crossing 310,839 cases alone, as of 30 April 2020

United Kingdom has overtaken France to become the fourth most impacted country, only behind the US, Italy and Spain, going by number of Covid-19 positive cases

The number of days taken to add 200,000 fresh cases to the global infected cases tally as well as the time taken to add 10,000 Covid-19 casualties is rapidly shortening. It took 122 days to reach the 200,000 mark since the first Covid-19 positive case was registered on 17th November in China and 67 days to cross 10,000 fatalities.

As of 30 April 2020, globally United States' share in the total death toll of Covid-19 was the highest in the world. In Europe, Italy's share then was 13.2% on 23 April but has since shrunk to 12% while France and Spain accounted for over 10% of the global fatalities.

Major reduction in the number of deaths has witnessed in Italy and Spain which were earlier accounting for almost half of the overall Covid-19 deaths in March. Apart from these four nations, the rest of the world accounted for almost 40% share in the overall daily death due to coronavirus.

At the time when the number of COVID-19 positive cases is rising by thousands each passing day, it is imperative to talk about the medical facilities that the countries currently possess. Ventilators are a much needed part of the facilities as they help a patient breathe when they have trouble breathing on their own. Countries across the world are short on ventilators but the country is all set to tackle this shortage. The data from China suggest that 15-20% of COVID-19 cases require hospitalization, with around 15% of cases presenting with severe symptoms and 5% requiring intensive care. In Italy and Spain, 40-55% of COVID-19 positive cases have been hospitalized, with 7-12% requiring admission to intensive care units. Estimates from China also suggest that patients in intensive care units (ICUs) require approximately 13 days of respiratory support, while data from Italy show that 10-25% of patients will require ventilation and some patients will need ventilation for several weeks. The deteriorating situation has raised serious questions about the level of critical care infrastructure across different countries, particularly that figure regarding ICU bed capacity. A study from the Chinese Center for Disease Control and Prevention found that while the vast majority of COVID-19 cases were mild up to that point, 13.8% were severe and 4.7% were critical. If the number of cases continues to climb at its current pace, intensive care units across the world will come under massive pressure. In order to show where that pressure could be highest, the following figure pulls together data from different sources to show the number of critical care beds per 100,000 inhabitants in different countries.

As the COVID-19 pandemic expands worldwide, with the United States now having the highest number of cases and exponentially increasing numbers of cases and deaths daily, the shortage of supplies is becoming paramount. The rapidly growing shortage of ventilators is also becoming of dire concern. Last year, 77,000 new ventilators were enough to meet the market demand of the entire planet. In April, New York City alone forecasts a need for 30,000 additional machines. Some ventilator manufacturers have already boosted their production by 30-50% but, by themselves, can't deliver the 500 or 1000% growth in production required. The top ventilator manufacturers-Gelinge, Hamilton Medical, Drager, Mindray, Medtronic, Lowenstein, Vyaire Medical, Philips, GE Heathcare, and Fisher & Paykel Healthcare, among others are today scaling up their production to meet the soaring demand. In addition, automakers have also announced they've been teaming up with existing ventilator makers to help them ramp up production. And some, like Ford, French automobile group PSA, Toyota, Maruti Suzuki, Mahindra & Mahindra, and GM are exploring producing ventilators in their own factories.
